Case Management & Counseling

GWTW offers Case Management and Counseling Services to women ages 14 to 44 that promote optimal health both physically and mentally. Services are offered by way of a women’s entry into the program, but are also offered to children and additional family members if needs are identified. GWTW services center around strength-based individual and family assessments that assist in identifying the needs of the individuals or/and their families. GWTW uses validated instruments for assessments such as: Patient Health Questionnaire Depression Screening, Perceived Stress Scale, Health Literacy Assessment, Ages and Stages (ASQ) Child Development screen and the ASQ Emotional and Socials Scales screening. Additionally, each individual is assessed using a bio-psychosocial assessment tool to screen for current and historical mental health concerns. A Home Safety Checklist assessment is conducted in each home where there are children under three years of age. All women are involved in the development of an individualized case plan and/or treatment plan if necessary. Treatment plans and case plans are used as the guide for delivery of case management and counseling services.
